On 2 May 2023, SafeWork NSW inspectors attended a construction site where they observed a contractor working on a first-floor balcony without adequate fall protection.
After a SafeWork NSW investigation the defendant, Lloyds Building & Development Pty Ltd, was charged with a breach of section 33/19(1) of the Work Health and Safety Act 2011.
On 25 March 2024, the defendant was convicted by the Downing Centre Local Court and fined $7,000.
Scott’s Refrigerated Logistics Pty Ltd

On 6 July 2021 a linehaul truck driver, was operating a heavy vehicle between his Brisbane depot and a changeover point at Clybucca. The truck driver attempted to cross the road when he was hit by a vehicle sustaining fatal injuries.
After a SafeWork NSW investigation the defendant, Scott’s Refrigerated Logistics Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 22 March 2024, the defendant was convicted by the District Court and fined $800,000.
Metsquare Pty Ltd

On 28 August 2019 a worker fell through a penetration approximately 2.7 m onto capped reinforcement bars at Wollongong.
After a SafeWork NSW investigation the defendant, Metsquare Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 22 March 2024, the defendant was convicted by the District Court and fined $450,000
Marist Youth Care Limited

From 7 January 2019 to 30 September 2019, two female workers at residential care premises were exposed to a risk of suffering serious illness or injury arising from experiencing work-related violence.
After a SafeWork NSW investigation the defendant, Marist Youth Care Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 11 March 2024, the defendant was convicted by the District Court and fined $300,000.
Enviro Pallets Pty Ltd

On 11 September 2019 a worker was performing maintenance under a pallet making machine when a metal arm of a pallet turner struck the worker in the head causing fatal injuries.
After a SafeWork NSW investigation the defendant, Enviro Pallets Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 8 March 2024, the defendant was convicted by the District Court and fined $300,000.
Qantas Ground Services Pty Ltd

On 2 February 2020, a Health and Safety Representative directed workers to stop cleaning and servicing planes arriving from China during the COVID pandemic. Qantas Ground Services Pty Ltd stood down the Health and Safety Representative from his employment and told him not to attend work until an internal investigation was completed.
After a SafeWork NSW investigation the defendant, Qantas Ground Services Pty Ltd, was charged with a breach of section 104(1) of the Work Health and Safety Act 2011.
On 6 March 2024, the defendant was convicted by the District Court and fined $250,000 and ordered to pay compensation of $21,000.
